Residential Maintenance Website and Logo needed

I need a lead generation site to capture leads with. - Create Logo for homepreventivecare Home Preventive Care (we do home service repairs, preventative before they are problems) Logo will be simple nothing to complicated. - Need form to capture leads - Need Phone number on front page - Need email address on front page - Need pictures that are relavant to residential home, multiple home, people working on a home etc.... - Colors are bright orange, bright green and dark gray like [login to view URL] - We will be using all the text on the site and more - We like this website alot [login to view URL] (they are local in our same area so we can copy exactly the website but I would really like it to be similar to it. - Do just like [login to view URL] but switch sides with everything, like logo on the right side and contact info on the left etc.. - We dont have any content yet but will provide later - We will be adding video in the future but wanted to mention this because I would like a link or place to place it on the website - Will need a live chat feature - Need the website to be SEO ready with keywords.
