Peronalized Survey

The following proposal’s goal is to realize a personalized Survey (named “Digitale Check-List”) which could be upload on our Company’s website. The “Chek list” will have the following structure: - 11 Business’s areas, in each 2/3 questions (total: 32 questions). Every single question has 2 column of answers, every answer 4/5 possbilities (the answer columns are two “numerical” scales). - An initial premise and a legend section that it would be to user’s disposition in every Area (e.g. a “?” in every questions Group which where the legend appears). - Interactive functions (e.g. the user selcts that a question is a not active operation in his company, therefore the survey must hide the question with a light color) The Survey objective is to allow us make some targeted analyzes for our clients, so from the “Check-list” we need some automatic outputs: - The list of questions which the user has selectes as area not active in his company; - A reticular chart per Groups of questions which measures the lv of answer (average of answers selected dor each Area). - Summary table of the level of each answer for each question. These output’s elements must be produced for us in image format (e.g. png) or pdf format. We will send you the survey’s content, structure and layout by message.
