I would like to create a spreadsheet that will show employee's corrective action records. I need columns for first & last name, date corrective action occurred, type of correction action i.e attendance, performance, policy violation, other. I would also like to use checkboxes for the type of correction so instead of typing the word, you can just click the box to determine which type of corrective action. I also need a column to reflect the expiration date of the corrective actions. I have attached a sample of what I need but please feel free to freelance.
