Build a Website

This is a simple project, I want a website or page that uses MySql and PHP & With a bootstrap interface. Basically its medical chart to show what medicine was taken and when. I want a place where you can enter the name of the tablet and the time, I want the date to be automatic from the PHP Date variable or some other kind of date fetch thing. Then I want a nice table that shows what What time it was taken, What Date It was taken & What was taken. Also use a config php file separate from main, thanks.
