build a checkout page

I have a website that needs the checkout page fixed up, am using hikashop and only will use hikashop due to fact it allows me to get customers credit card info so i can input manually into my suppliers website. I need a checkout page that Will include the following 1- Calendar showing all the days of the month up to 1 year with ticket prices on calendar dates. see attached images. 2- set number of tickets 3- checkout process that is linked to hikashop where customers enters credit card info and that info is sent to my email using two separate emails ( its already implemented in hikashop).
