EASY DATABASE/FORM SCRIPT FOR EXISTING SITE

I need a module that will allow me to create a database of radio stations by name, location, website url, email address and phone #. I need to be able to easily add, edit, and delete entries. Secondly, I need my member website to be able to access this database by forms. I want to be able to activate certain radio stations to which members can fill out a short form, click submit, and the form is automatically emailed to that radio station's email address (in the database). The tricky part is I need sessions that will only allow the members to do this once per radio station, per 24 hours. The admin area for the database can be bare bones. I just need the script w/ directions. I can install myself. Need this within 48 hours max or 24 if possible.
