ASP Developer Needed for Tournament Registration Website

We have a sports tournament registration website that needs modifications. The current 'signup' form can be found here: [login to view URL] There is also a very basic admin interface here: [login to view URL] user/pass: aaron/king The main thing we need to have done is to change the email that is generated to the people who signup. I have attached a Word document to this bid request that shows the exact format needed. The email that is sent to the users will need to look exactly like the Word document. In the admin there is a feature to print out registrations in case an email is lost. This will also have to be changed to match the new format in the Word document. The second feature needed is a 'digital signature' waiver for people attempting to register. Currently we force users to check a box but we want to implement a waiver before users get to the form. It will not be very complicated. Basically we'll have them enter their full name into a box below the waiver text. Then we'll have a randomly generated number on the screen where they have to type in to confirm it. This information will be logged to the database along with a timestamp. The current database is MS Access and all code is ASP/VBScript. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
