Create design for GuestBook Script

I am looking for design for my guest book script that I have setup. I am basicly turning the script into a testimonial script, and I would like a nice design. default script setup: [login to view URL] I like this design format, if you can do better great... (link at bottom) I would like an image: Submit Testimonial links across top of page. user submit on left. on the right, where you see text, and links..I would like that area reserved for a youtube video..where I can just copy/paste the code. and I would like the user posts to come out sorta like what you see on this site. the site design does not have to be all one solid color.
