Form to upload files and create a xml or html config file for video player

I need a second page on the 'insert new medicine' form. This form captures various bits of info and inserts as a new product into a table called 'medicines'. The 2nd page of the form must do the following: 1. create a sub-folder in the 'videos' folder in joomla root 2. allow a user to upload 7 selected video files and 7 jpg thumbnails (large-ish uploads - 10-20megs - something stable and that will refresh joomla login?) 3. take the values for the names of the 7 videos and thumbnails, and the new folder name, and output along with pre-printed text in to a html file which is placed in the created sub-folder also 4. set the name of the created folder also as a field for the record being entered by this form in the 'medicines' table 5. copy 2 additional files, a .css and an image file, in to the created folder as well I will send you copies of the existing tables and the form html code. This form is written using Chronoforms on Joomla, so you will want to have experience with, or do a test install of, both. The winning bidder will have a competitive offer, will indicate how soon they can start, what (if anything) they need besides copies of the tables and the form html, and will also let me know how soon (approximately) they can have the code ready. I will want you to send me the html code for the form with which I will replace my existing form html. I will test the code myself to make sure it works. I'm not worried about how it looks, I can change that myself.
