PHP & Html Lead generation platform

I'm looking to build a site that will allow me dispatch leads to multiple companies based upon each companies location. I want to build a system that will dispatch and record where each lead goes. This system will also keep track of which company it's sent leads to, date, time etc. This system will allow me to create a company profile and assign cities to the company and an email address id to the company. I need to be able to have a city template that will allow me to change fields from within the template through the database. I also need to be able to create 50,000 unique city pages by uploading the data from an excel spreadsheet. I also want to create unique pages that are populated from a 2nd excel spreadsheet. The system will calculate the # of leads dispatched to each company and it will email them at the end of each month a request for payment via email. I need this system to allow affiliates/partners to sign up to be an affiliate and then it will put them into a que for approval. I have a model of the site already working I just want to perfect it.
