URGENT: TODAY: Landing Page/Order Form Coding

Hello, I will be awarding this project TODAY (within just a couple hours of posting- as soon as the first bid I like comes in). I have the graphics in place already for this and now I need a coder! I need you to code a website which gives away a magic trick and then gives the option to purchase more magic at a special price. Consider the type of landing page and order form similar to a Free Trial for a Weight Loss, Acai, or other product. My Free trick does not require shipping at all. Upon acceptance, you will take the PSD graphics I have and create 4 pages. The first page will ask for the following: Child's First Name, Child's Last Name, Birthday, Parent's First Name, Parent's Last Name, Email, and Phone #. The 2nd page will pass along these variables and ask for the next group of information: Ship To: Address, City, State, Zip Code. The 3rd page will say, "Before we send your Free Wand, we'd like to give you a Free 1 Month Membership to The Young Magician's Academy, just pay $3.00 S&H to have it sent to you." This page will then ask for the following info: Billing Address, City, State, Zip, Credit Card Type, Credit Card, Exp. Date. 4th page will be a thank you page, telling them we have received their info and that it will ship tomorrow for them. What I need you to do is integrate the following pages you make with my Infusionsoft account. I will give you the web form code for the first 2 pages (you'll need to CSS and modify the LOOK of them) plus the order form code for the 3rd page, which you'll need to modify the look of too with some CSS so it fits within the look of the site and not just the standard form) and then of course add the handlers to accept the pass through data. Infusionsoft forms have an option to pass the data to the next page, you'll just have to make sure the pages receives it and prepopulates the next form. This needs to be completed within 1 day, so please include the words "MAGIC TODAY" in your bid or else it will be ignored. I need to know you can complete this TODAY!
