Need Ecommerce Order form incorporated into website

I need an ecommerce page designed for a website client. The site is finished but needs to have an order form designed for the website that can be processed on the site. The client has cyber strongs "eshop" shopping cart and needs to integrate this with paypal pro application. The client also needs to make certain that they can manage this later or make updates. Need a developer familiar with paypal's system and intergrating this with shopping cart software. Order form will need to be able to process a few questions as well when order is submitted. The client would like something that looks like this ... but not as big or advanced ..
