Website and MYSQL Database Site

WEBSITE REGISTRATION PROJECT We have a project that needs a web site with about 5 to 8 forms and a Mysql database of registration records. There is a need to create records as well as be able to recall the registration records. The general field contents of the records are in a design document. I have created a document that lays out the general page designs, the menu bar and items. I want a standard header/menu/footer on all pages. It is a straight forward website and data that should be quite easy for anyone experienced with a website to do registrations and that create a mysql record. The website can be an html and php composition. The database is mysql. There will be some multi-user access to the database – that should be addressed. There is planned large scale high volume internet access and record creation will be a phase II for the project. Database – There are 2 record types. 1. The registrant (owner) 2. The item and its associated number. There is a need to be able to pop and show the item record when the registration number is entered into a form or when there is a URL to the site with the item number as part of it. The item number is 12 alpha-numeric digits There is a need to have a 2 digit checksum on this number to ensure correctness and prevent random entry of numbers.
