crowdpay api integration --very urgent need

Project Description I am looking for a simple PHP script for school classrom. So as i said i need php script which will have admin, teacher and student login (user interface) Script will monitor condition of computer for every class and student. This means that every student will login in app and write problems with computer for example: -> damage on computer from previous student, trash stuck in computers place and computer system problems. I only need simple html and css design and fully working php and mysql script. Also there must be option in teacher interface to import students with .csv file and there must be also a statistic so you can check for every computer the problems (from date to date) Thanks
