Email Import Script

I need a script to import all emails addresses into an email table in my database. I have one csv file with email and first name. I have one text file which contains all email addresses. The script needs to have the option of importing the email address and/or first Name into table. You can choose to run script with phpMyAdmin, SSH, or a php file. Please provide detail instructions. I will put money in escrow, and will not release money until I am able to successfully import email addresses.
