Need an urgent website on patient management system based on HTML, PHP, My SQL and running on XAMPP

I am in urgent need of a web developer who can develop a patient management system. Basically the website will be a simple website of not more than 3 to 4 pages and will minimal in design and functions. It will help patients to first register at the site and upon registration, the patients can then login at the site and then can book the slots accordingly for the doctor. There will be two components - one will be the user page wherein registered users can book the slot for them for next 7 days and the other component will be the admin page will enable the admin (the doctor) to login to the page and then manage the patients. The admin can view the patients registered at the site and the number of patients who have booked the slots. He can also delete the patients from the list of patients if needed. The website needs to be made in HTML, PHP and should be connected with My SQL in backend. It needs to be hosted locally on XAMPP as live hosting is not needed. So the website will run locally on desktop/laptop under XAMPP (not WAMPP or LAMP). The project is needed urgently in next 3 days and by tomorrow afternoon, the user login page is needed. Later on other components can be added to the page and site. The code for the website cannot be copy pasted directly from the internet but needs to be unique. The code will be checked for copy pasting on copyscape and other copy paste detection sites and if there is more than 30% duplicate code then the project will be cancelled. So pls bid only if you can provide original code without copy pasting from net. Content for the website will be provided by us once the right bidder is selected
