Directorate general of health services Al Dakhiliyah Governorate Documents Verification

The task is to visit the location to: 1- Verify three documents and get a three form filled and sign stamped by the Directorate general of health services Al Dakhiliyah Governorate. 2- 3 to 4 geotagged photos are required with health centre name board visible. 3- In case of refusal take the written refusal with stamp with details of the person who refused to fill the for line name, designation, phone number and email. If above mentioned requirements are fulfilled than task will be completed.
